Vintage Faire is located at the north end of Modesto, across the street from Vintage Faire Mall with adjacent retail stores and restaurants. Vintage Faire is a 99-bed facility licensed for Medicare, medical and HMO patients for both short and long term residents. However, our current focus is short-term rehab and we are not accepting long-term residents at this time. We are currently a staff of eight, with PT, RT, OT and ST. The team is an excellent mix of experienced and newly graduated therapists. The programs we have in our facility include: Orthopedic Rehab, Falls Management, Wound Care, Pain Management, Contracture Management, Splinting and Positioning, Low Vision, Continence, Homeward Bound, Seating, and Cognitive and Respiratory Specialties. Training for these programs are provided during the orientation process and on a quarterly basis. The patient group we see at Vintage Faire is a great mix of age groups and conditions due to the variety of payor sources accepted. We have contracts with seven different HMOs and meet weekly with the case managers to ensure quality of services. The philosophy of our therapy department is definitely to enjoy coming to work and to support each other wherever we can. Because we are a small group, we are able to work very closely with all disciplines to achieve the highest goals for the resident. In additional to all the routine features of a Rehab department, we have a Transitional room (our Homeward Bound Suite) which allows us to ensure residents are ready for discharge, and in the gym we have a fully set up bathroom area to perform detailed assessments and demonstrate equipment to both family and residents. We also have an out-patient license so we are able to invite patients to return to us on discharge to complete their therapy on an out-patient basis and also this allows us to accept residents from local assisted/independent living centers who need therapy services. All in all, we are a great team to work with and have been able to develop an excellent service record through the efforts of the entire therapy team.

Required experience and qualifications:
Bachelor's degree in Speech Language Pathology from an accredited school.
