Provides and directs Physical Therapy services to residents to include assessment and treatment, development and implementation of programs, and related professional in-services. Performs duties under the direction of physician?s orders. Responsibilities: - Provides Physical Therapy services to residents and initiates referrals when appropriate and in accordance with Physical Therapy Department procedures.
- Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
- Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
- Interprets and communicates evaluation findings to residents, family, and other team members.
- Monitors resident?s response to intervention and modifies treatment as indicated to attain goals in accordance with physician?s orders.
- Participates in discharge planning including programming to maintain and enhance the performance of the resident in his or her own environment.
- Documents results of resident?s assessment, treatment, follow-up, and termination of services.
- Reviews the quality and appropriateness of the total services delivered and of individual Physical Therapy programs for effectiveness and efficiency, using predetermined criteria.
- Supervises Rehabilitation Aides, students, volunteers, directs LPTAs in the implementation of the treatment plan
- Assumes responsibility for ongoing continuing education and professional development
- Complies with all Infection Control, Universal Precautions and OSHA standards for the health care professional.
- Assists with and participates in Quality Assurance Programs as required.

- Successful completion of National Certification Examination for Registered Physical Therapist.
