We are looking for a Registered Nurse to fill a permanent full-time Community Liaison position in Nashville, TN for a Home Health company. JOB SUMMARY:
The community liaison is responsible for educating doctors, discharge planners, social workers and potential clients in the benefits and services provided by home health.
REPORTING RELATIONSHIPS:
The Community Liaison is directly responsible to the Director and Administrator of the Home Care Agency.
EDUCATION:
Tennessee RN License Required. Associates or Bachelors degree in nursing
EXPERIENCE:
One to two years work experience in community education related to long term care, assisted living, home health, and/or related settings preferred. Excellent communication and interpersonal skills preferred. Working knowledge of Medicare eligibility requirements preferred. Ability to work in an autonomous environment and time management skills preferred.
JOB DUTIES/RESPONSIBILITIES:
1. Conducts educational in-services with community organizations.
a. Plans, delivers, and completes written summary of all educational in- services.
b. Develops relationships with discharge planners, social workers, clinic managers, doctors, etc. in an effort to improve the appropriate utilization of home health services.
c. Educates community organizations on the importance of home health as part of the continuum of care.
d. Participates in community health fairs
2. Monitors and evaluates effectiveness of educational programs.
a. Proactive in developing marketing and educational literature.
b. Maintains database of all current and potential referral sources.
c. Provides director with feedback regarding the effectiveness of current educational and marketing programs.
d. Completes customer service questionnaires in order to determine level of patient satisfaction and provides feedback to agency director on all quality of care issues.
