| Position Description: |
Full time LVN Clinical Coordinator I for our Santa Fe Springs, CA branch. Understands the overall operations and functions of a home health program. Assigns referrals for new patients. Assesses daily staffing needs and notifies nursing personnel accordingly. Acts as liaison between the physician, staff and patient/family. Demonstrates flexibility in a changing work environment. Informs Director regarding specific patient needs, complexities or unusual findings to promote continuity of care. Submits requests for additional visits prior to expiration of current authorization. Participates in daily report with Pharmacy.

This position requires working cooperatively with all levels of professional staff. Attention to detail and follow-thru are a must. |
| Schedule: |
Monday - Friday, 8am-5pm
|
| Experience Required: |
Must have current CA LVN license and a minimum 2 years experience in a home health agency or equivalent. Experience with infusion therapy preferred. Nursing staffing or home health staffing experience is a plus. Clinically-oriented (acute) LVN's with IV infusion experience will also be considered for this position. |

$18-24 per hour |
