Pharmacy Clinical, Conroe, TX
ICU Clinical Pharmacist
Shift Tues-Sat days
Under the supervision of the Director of Pharmacy, the Pharmacy Clinical Coordinator:
Coordinates ongoing Pharmacy activities, which promote appropriate, safe and cost effective drug therapy with the hospital.
Manages and monitors drug utilization through the use of a formulary and the use of a formal, systematic process for tracking drug use.
Promotes and enhances Pharmaceutical Care practice by providing appropriate leadership, management and education. Serves as a role model and educator for the staff in terms of pharmacy practice and patient care.
Coordinates the Medication Use Evaluation {MUE} program to improve patient care outcomes and ensure adherence to all JCAHO or other regulatory agencies. The MUE program shall be well organized, adhere to the FOCUS-PDCA format, and involve the entire pharmacy staff.
Promotes optimal medication use through participation in multi-disciplinary activities and education of other healthcare professionals.
Assists the Director in identifying and establishing specific goals and objectives, which are written, measurable and includes a specific time line. Goals must be established for high cost utilization, and professional/personal achievements.
Demonstrates personal/interpersonal skills which fosters quality, teamwork and customer service.
This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. They must also be able to demonstrate the special skills required to care for patients and their families from birth to death according to standards.
Licensure/Certification/Registration: Current Texas Pharmacist License
Education: A BS in Pharmacy with clinical skills or a Pharm.D. Degree
Experience: Three to five (3-5) years pharmacy management experience preferred
Knowledge/Skills/Abilities: The Clinical Pharmacist should have adequate clinical pharmacy skills including development and operation of the medication use program, adverse drug reaction reporting, medication variance reporting, development of critical pathways, investigational drug services, protocol review and development, as well as organizational and communication skills. I
Internal Contacts: Physicians, nursing personnel, various other departments utilizing pharmacy services.
External Contacts: Patients, vendors' representatives, pharmacy students, federal, state and local regulatory agencies.
