Our skilled nursing and subacute facilities are located in 9 states: Massachusetts (Medford, Holyoke, Westfield), Vermont (St. Johnsbury, Bennington, Burlington, Barre, Springfield), St. Albans) New Hampshire (Rochester), Rhode Island (North Kingstown ), Connecticut (Wallingford, Waterbury, Bristol), New Jersey (Wayne, Bloomfield, Passaic, Somerset, Toms River, Brick, Whiting, Linwood, Whiting, Williamstown), Virginia (Dunn Loring), and Washington State (Tacoma, Puyallup, Montesano) and Maryland (Silver Spring).
